Gifts Remittance Authorization Form

I hereby authorize the Baptist General Convention of Oklahoma, hereinafter called Company, to initiate debit entries to our church checking account and the bank indicated above. These drafts will only be processed for the exact amount that was authorized. This authority is to remain in full force and effect until Company has received written notification of the church’s termination of agreement in such time and in such manner as to afford Company and Bank reasonable opportunity to act on it.
