Download the two forms below (W-9 and Workers Comp). Please note, while our office requires a completed copy of the Workers Comp affidavit in order to process payments, we do not require you to file the Workers Comp form with the Workers’ Compensation Commission. Fill those forms out and return them to Carissa Jones (cjones@oklahomabaptists.org).
We cannot pay you until these forms are received.
